Families who would like their child(ren) to change their mode of education (Hybrid to Fully Remote OR Fully Remote to Hybrid) starting second semester (January 19th), should fill out THIS FORM.  The Request Form will be open until January 6th at 7:00 PM.


NO ACTION IS NEEDED IF YOU DO NOT WISH TO REQUEST A CHANGE.
